Quick Links
You’ve just defeated the Flesh Lip boss and finished up the longest single area ofSilent Hill 2, Brookhaven Hospital. Congratulations. Except you’re not really finished Brookhaven yet, not entirely. Welcome to everyone’s worst nightmare - being stuck in a hospital you’re not allowed to leave.
The Otherworld version of Brookhaven Hospital is nowhere near the scale of the real-world version, though that doesn’t mean this run will be easy. Brookhaven is dramatically altered, and it all centers around opening this one little box. Brookhaven has plenty of surprises for James in store.
The article was written using the Normal Puzzle Difficulty, and so the solutions may not match the Easy and Hard difficulties.
Getting To The Second Floor
As soon as you enter this Otherworld Brookhaven, you will thankfully still retain your map, though it acts slightly differently this time around. It will become splotchy and darkened as you explore, leaving a trail on the map everywhere that James has walked. In a way, this makes it easier to actually track where you’ve already gone.
You’ll start on the first floor of Brookhaven with the goal of reaching the second floor. This section of the first floor you’re on is quite restricted. On the way to the elevator, you’ll find someHandgun Ammo and a Health Syringe, as well as a red save point. After picking these up and saving your game, you’re able to enter the elevator.
Your only other option right now is the second floor, with the third floor button broken and the basement button missing.Head on up to the second floor, where the real fun begins.
As you enter into the lobby of the second floor, you will be greeted bya massive chained box, itself covered in even more locks. Someone really wanted to protect whatever was inside. This is the main goal of the Otherworld Brookhaven. Save your game at the red save point on the wall at the back of the room, and we can get started.
Removing The Combination Lock
You can choose to go for either the Combination Lock or the Keypadto remove first, but we elected to remove the Combination Lock first. This puzzle is restrictedexclusively to the second floor, so it can be completed fairly promptly.
The aim here is to ultimatelyreach the room M2, in the south-west corner of the second floor. We have to go through a few hoops to reach here though, so let’s get started.
The north-west corner of the second floor is almost entirely optional, and we have a dedicated section on all of the items there further down.
First, we want to go to M1, the room directly north of M2. To start, go directly south of the Chained Box in the direction of the Director’s Office. You’ll then have to cut through the Restoom, and again until you reach room L1. In here, you’re able to find aHealth Drink and some Handgun Ammo. Pick them up up, then proceed to the left into M1.
First, pick up the nearby box ofShotgun Shellon the window. On the wall of this room, you will find a rusty clock. Once you straighten out the hand by interacting with it, you can turn it around to whatever time you want. The goal is to set it to 2, though verify you’re prepared first. Once it hits two, an alarm will sound, summoningapproximately three Bubblehead Nurses. Once they’re defeated, proceed to room M2.
South of M1, you’re able to enter room M3 directly, picking up theHandgun Ammoinside. Break the wall here to enter into M1. Here, you will find someHandgun Ammo, as well as a diary that says the following:
“1 year they’ve been watching me
So many pills they fed me
When a loud hours comes
I tremble every time the light falls”
This is your hint for the code to remove the Combination Lock.1 is the first number. You can examine the sink in this room to check the number of pills.6, so that’s your second number. When a loud hour comes refers to the clock, which you can examine through the door. It’s at 2, so that’s your third number. The light is slightly more complicated, but refers to the amount of times it flickers each time it dims.It is twice. So your final code is 1622. You can now return to the lock to remove it.
Before this, enter room M5to examine the wall for a Glimpse of the Past, as well as someHandgun Ammo.
Now we can move ontothe Keypad.
Unlocking The Keypad
While you may have already entered it on your way to M2 for the Combination Lock, you’ll want to enter room L3, which you have to climb through an open window to get into. Inside, you will find someShotgun Shells and Handgun Ammo. Now move to the Observation Room.
In here, you will find thePatient’s Note, the ‘No One Knows’ Strange Photo, and the Director’s Office key. You can now continue down the hallway south until you reach the Director’s Office.
Check into the Restroom along the way to pick up aHealth Drink, as well astwo boxes of Handgun Ammoin room C2. You can also break a wall here to enter into the Shower Room and through to room L2, which opens back into the lobby. You can find someHandgun Ammo and a Health Drinkin these rooms.
Unlocking the door to the Director’s Office, you will see it is quite changed. You can find someShotgun Shells and a Health Drinkin a nearby cabinet, as well as a tape recorder to listen to in the same location as the original Brookhaven. After finishing, climb the ladderby the desk up to the third floor.
You will find an electrified stretcher up here, with the goal ofrestoring power to it. This is quite a linear section without any puzzles to solve. You just have to make your way to the room north of the Restroom, activating the generatorinside. You can findtwo boxes of Handgun Ammo, a Health Drink, and a Health Syringealong the way.
Return to the stretcher and activate it for the code to the keypad to appear on it.9659. Return to the Keypad and enter this to unlock it.
Collecting The Lapis Eye Key
But of course, we’re not finished yet. With the chains removed, you mustnow find the actual key for the box. To do so, you must return to the third floor, this time through thenewly opened staircasein the north-west of the second floor.
Here, you mustgo to Exam Room 4to find a mannequin guarding its face. Beside it you will find theInstrument of Force, which you must use to remove its first arm. Now, you must head to room D3, andtake the ladder there downto the first floor. Make sure to pick up theHandgun Ammo, Health Drink, and Shotgun Shellsalong the way.
On the first floor, you’re able to findfive boxes of Handgun Ammo, three boxes of Shotgun Shells, a Health Drink, and a Health Syringestrewn about. In the Restrooms, you can findthe Key of Bliss, needed for the Mannequin on the third floor.
Return to the mannequin, moving its arm out of the way andpiercing its eye with the Key of Bliss, rewarding you with theLapis Eye Key. Return to the Chained Box and open it to reveal James' grand prize -nothing.
Once you open the Chained Box, the design of Brookhaven will change, locking you off from many previous areas. Make sure you’ve explored everything you want to before opening it.
Dismayed at the lack of anything in the box, his only compensation is the sudden materialization of a wall behind him with a maiden in a red dress, the elevator button for the basement in her mouth. After taking it, you will notice that two rings may fit on her hands. This will be found in the Basement and other new areas accessed through it.
Optional Areas On The Second Floor
Before proceeding further, you should take the time to explore any areas on the second floor that you haven’t yet. While you will have found the majority of important items while solving other puzzles, there is ahealthy dose of ammo and healing items still to be found.
In the Treatment room to the west of the Chained Box you will find someShotgun Shells and two boxes of Handgun Ammo. Proceed to the Nurses' Lounge to find aHealth Drink and another box of Handgun Ammo. Further south in the Women’s Locker Room, you will findShotgun Shells and a Health Syringe.
With these acquired, you arefree to proceed onward.
Finding The Rings
Once you start taking the elevator downwards,a quiz show will start playing out of nowhere. You don’t have to do anything right now, though pay attention to what it being said asit will never be repeated. Mind you, if you don’t already know the answers, hearing the questions won’t change anything as they are based on information that is not shown in James' notes for the most part.
The Copper Ring
In the Basement, your goal is tofind the Copper Ring in the Pump Room. There’s nothing by ways of puzzles here, so explore around to find a wide assortment of items before going to the Pump Room last.
In total, you can findfour boxes of Handgun Ammo, two Health Drinks, and a box of Shotgun Shells. Once you’ve picked all these up, roll the laundry basket to the back of the boiler room and climb through the open section at the top of the wall to enter the Pump Room, picking up the Copper Ring.
Suddenly,Maria reappears, James and her delighted to find each other in this twisted place. Maria has now joined you in your search for the second ring, so make sure to keep her safe.
Proceeding up the stairs, you will be back on the first floor. The aim is to reach the Doctor’s Lounge, where you will find the Lead Ring. This is a mostly linear section as well, with a few items as well as a Strange Photo to find.
AHealth Syringecan be found in Exam Room 3, while theStrange Photo ‘They Mustn’t Know’is found by the north-west edge of the pool. These are the only optional items to be found, so proceed on to the Doctor’s Lounge.
After opening a floored refridgerator filled with viscera most vile,Maria will fondle the Lead Ring, thinking it’s nothing special. After that brief cutscene, you are free to return to the second floor through the elevator in the hallway just outside of the Locker Room. Pick up theDissociated Noticehere, and then save your game before taking the elevator back up.
If you think you know the answers to the quiz that were asked to you in the elevator down to the basement, then head into the newly-opened pharmacy. The aim is to input the answers into the chest here in the order they were asked. Don’t worry if you can’t get it, it’s only some ammo and healing items.
Escaping The Otherworld Brookhaven
Back by the relief of the Maiden on the second floor, slide the rings on to her fingers.Place the Copper Ring on her upper hand, and the lead ring on her lower hand. With the rings placed, the relief will open into a passageway, a dark hallway into the depths of Brookhaven. This is your very last chance to explore, so save before going any further.
Once you enter the passageway, you can pick up a note simply titled ‘Ominous Note’. Continue onward until the specter chasing James appears once again -Pyramid Head. This will start a chase sequence with the pair of you running from Pyramid Head. Try not to look back.
Once you reach the elevator at the end, things take a turn for the worst. We won’t spoil them on you, butJames is left to leave Brookhaven by himself. Once he exits the elevator on the first floor, you can pick up aHealth Drinkin the Examination Room, and theHospital Entrance Keyin the Reception Office.
Save your game at the red save point, and then leave Brookhaven Hospital intothe Otherworld version of South Vale.
